Please use this form to request to have your book transformed into a special edition! After submission, we will follow up with further instructions, finalize colors and cover design, and work closely with you as we prepare your Special Edition.

Please get an accurate weight of the order before purchasing. Any additional fees due to inaccurate weight measurement will be covered by the buyer. Indie Reads will generate the shipping labels. Please ONLY ship books, no other items.

Please provide a description of desired cover design, both front and back, keeping in mind that these are cut-out, iron-on designs, not graphic art.
